- Member’s use of the club card indicates acceptance of the conditions of the A-Play Club Card.
- Rules of the program may vary with specific promotions.
- All individuals / cardholders must be twenty-one (21) years of age or older to participate in A-Play Club Program.
- Accounts with no play after 24 months will forfeit all points earned.
- Membership in the A-Play Club is a privilege and may be revoked or cancelled at management's discretion at anytime. All cards must be returned at time of revocation or cancellation. Club members' points may not be brokered, bartered, or sold and may not be transferred as part of a civil or domestic relation matter. In the case of a spouse death a certified copy of the death certificate must be presented to Lakeside Casino to keep on file for the points to be transferred to the surviving spouse.
- Valid photo identification (i.e. Drivers License, passport, State I.D., Military ID) is required for ALL transactions pertaining to A-Play Club.
- It is the member's responsibility to properly insert card when playing Slots or Video Poker machines and to present card to the appropriate personnel when playing table games or when purchasing gas, meals, hotel rooms, groceries or merchandise.
- Members may not distribute, load or in any way allow another person to use their A-Play Card.
- Lost or stolen cards should be reported at a club location immediately. Lakeside Casino is not responsible for lost, stolen, or misused cards, or for points redeemed on it. Lakeside Casino is not responsible for lost or stolen redemption vouchers.
- All disputes and ensuing decisions by management regarding point accumulation or point balance are final and binding. Lakeside Hotel & Casino management reserves the right to adjust any point balances resulting from machine malfunctions, operation errors, and/or fraud. Account adjustments will be completed within 72 hours of being validated.
- Player's club members who have been issued credit and are delinquent in repayment of that credit and/or members who have had a check(s) cashed which have been returned for non-payment shall have their Players Points forfeited until all discrepancies have been remedied.
- Membership benefits are non-transferable and cannot be used in conjunction with any other program or promotion unless otherwise stated.
- Any dispute or situation not covered by these rules will be resolved by management in a manner deemed fairest to all concerned and that decision shall be final and binding. Lakeside Hotel and Casino management reserves the right to adjust any point balances resulting from machine malfunctions, operation errors, and/or fraud. Account adjustments will be completed within 72 hours of being validated.
- Unredeemed point balances are not transferable to any other account.
- Only the cardholder may redeem points off their accounts.
- Employees of Lakeside Hotel and Casino, Affinity Gaming, E.T.T. and their immediate families are prohibited from participating in any Player's Club contests or drawings.
- Point chasing or point theft is a fraudulent use of the club card. Point chasing is defined as allowing individuals other than the cardholder play on your club card. Members may be expelled from the club for such action and all Player's Club privileges may be permanently revoked, including elimination of all remaining point balances. Any other misuse of the card will constitute automatic grounds for expulsion from the Player's Club, including the elimination of point balance.
- Accounts with no activity for twelve (12) consecutive months will be considered inactive and all points will be forfeited.
- Any A-Play Club Card, Voucher, or drawing ticket that has been mutilated, forged, misprinted, altered, tampered with, mechanically reproduced, is illegible, or has been obtained by any means other than pursuant to these official rules is automatically void. In addition, Lakeside Casino reserves the right to void any Player's Club Card, voucher, and drawing ticket at any time in its sole discretion.
- If an A-Play Card is removed from the machine prior to completing the current session, the point will be lost.
- Management reserves the right to adjust any point balance resulting from malfunction, operational errors and or fraud.
- Use of A-Play Club card constitutes acceptance of all rules, terms and conditions stated.
- Employees of the Lakeside Casino / Affinity Gaming corporation are not eligible to participate in this program.
- Lakeside Casino reserves the right to modify or cancel this program, including all benefits, point values, and redemption values at any time.
